chanserv: op, deop, voice, devoice, kick, ban, unban

Adds channel moderation commands sourced from the ChanServ pseudoclient.
Tracks connected users' nicks and hosts so targets can be resolved by
nick and banned by *!*@host. All commands require operator access.
