auth: offload IDENTIFY + SASL PLAIN verify off the engine lock

Both ran scram verify_plain (~1s PBKDF2 at 1.2M iters) inline under the engine
lock, freezing the whole daemon per login. Add NetAction::DeferAuthenticate +
AuthThen continuation + ctx.defer_authenticate, mirroring DeferPassword: the
module/SASL path fetches the verifier cheaply and defers; the link layer runs
verify_plain on spawn_blocking, then Engine::complete_authenticate finishes the
login (IDENTIFY reuses the same ctx helpers, so its login/AJOIN/vhost/memo
side-effects are unchanged). Tests resolve the defer inline (cfg(test)). The
gRPC web-login path was already fixed; no login path stalls services now.
(GHOST/DROP/CERT/GROUP still verify inline but are rare account ops, not logins.)

services: throttle emailed codes to stop RESEND/RESETPASS email-bombing

REGISTER was rate-limited but RESEND and RESETPASS weren't — anyone could
repeatedly request confirmation/reset codes for any account with an address
on file, flooding the victim's inbox and burning the service's sender
reputation. A per-account 60s cooldown (code_issue_wait, mirroring the vhost
request throttle) now gates both paths; guessing was already infeasible
(2^40 code + 5-try limit), so this closes the abuse, not an auth hole.

OperServ: enforce FORBID EMAIL at registration and SET EMAIL
FORBID accepted and stored EMAIL patterns, but nothing ever checked them:
a forbidden address still registered and could be set via NickServ SET
EMAIL. Both paths now reject a matching address (new ForbiddenEmail
outcome), matching how NICK and CHAN forbids already gate registration.

NickServ: add SET HIDE STATUS for last-seen privacy
Lets an account keep its last-seen/online line in INFO visible only to
itself and to opers. Default stays public (Anope's default). Accepts
STATUS (with USERMASK as an alias) per Anope's SET HIDE <field> grammar.

NickServ: show last-seen (and online status) in INFO
The account's last-active time was tracked but never surfaced. INFO now
reports it, reading 'now (online)' when the account has a live session.
Threads NetView into the INFO handler for the session lookup and carries
last_seen on AccountView.

NickServ: add SET KILL to toggle nick protection
Nick protection was unconditional. SET KILL OFF lets an account opt out —
an unidentified user keeping one of its nicks is no longer prompted or
renamed to a guest. Stored inverted (no_protect) so protection stays the
default. Accepts Anope's ON/QUICK/IMMED/OFF; grace is a fixed interval
here, so the finer variants simply enable protection like ON.

NickServ: add SET AUTOOP per-account auto-op opt-out
A user with SET AUTOOP OFF is never auto-opped on join, even where they
hold channel access — they op themselves via ChanServ UP. Stored inverted
(no_autoop) so the default stays auto-op. Join gates on both the channel
setting and the account preference; either can opt out.

MemoServ: add SET NOTIFY and SET LIMIT

SET LIMIT <n>|NONE sets your mailbox cap (honoured on SEND, NONE = the

while RECOVER also puts you back onto it (frees the ghost if present, then
SVSNICKs you to the nick). RECOVER works on an already-free nick too.

ChanServ: add SET SUCCESSOR with founder inheritance

SET <#channel> SUCCESSOR <account>|OFF names an account that inherits the
channel if the founder's account is dropped or expires, instead of the
channel being released. Adds a ChannelInfo.successor field + event, and
centralises the founder-gone channel release into a single
release_founded_channels store method used by DROP, expiry, and the
gossip account-gone path so all three transfer consistently.

Front the whole network's help through HelpServ
Each service now exposes its help catalog via Service::help_topics; the
engine collects them into a network-wide index on Network, reachable
through two new NetView methods. HelpServ uses it: HELP <service>
[command] (or a bare service name) serves any service's per-command
help, bare HELP lists the services, and its own ticket commands still
work. No cross-crate dependency: the catalog flows through the SDK's
NetView, keeping every module dependent only on echo-api.

Add centralized HELP with per-command subcommands
New echo-api primitive: a HelpEntry table plus a shared help() renderer.
HELP lists a service's commands; HELP <command> prints that command's
detail. Every service routes its HELP arm through the one renderer, so
the shape is identical across the network. Wired into NickServ and the
six newer services so far.

Rename the project to Echo (was fedserv)
Rebrand the daemon and workspace: the binary is now `echo`, every crate
is `echo-*` (`echo-api` and the module crates), Rust imports use
`echo_*`, the gRPC proto is `proto/echo.proto` with package `echo.v1`,
and the log filter, gossip peer-name default, and on-disk store default
(`echo.db.jsonl`) follow. Docs updated throughout. No behaviour change;
crate directories and the config schema are untouched.

Group the module crates under modules/
The service pseudo-clients and the ircd protocol link sat flat at the
repo root, mixed in with the daemon core and the SDK. Move them all
under modules/ so the tree separates concerns cleanly: the daemon in
src/, the SDK every module links against in api/, and the loadable
modules — the pseudo-clients plus the protocol link — in modules/.

Workspace members, the daemon's per-crate dependency paths, and each
module's api path are updated to match; the docs follow. No code change.

modules: split services and the ircd link into external crates
nickserv, chanserv and the InspIRCd protocol move out of the binary into their
own workspace crates (fedserv-nickserv, fedserv-chanserv, fedserv-inspircd),
each depending only on fedserv-api. human_time and the branded account emails
move into the SDK crate so a module needs nothing from core; the engine keeps
its own inherent methods and builds emails via fedserv-api too. The bin now
constructs each module from its crate instead of an in-tree #[path] include.
Proves the SDK is self-sufficient: a third-party module is the same shape.

modules: capability-scoped Store/NetView boundary (Tier 2)
Services no longer receive the concrete account store. on_command now takes
&mut dyn Store + &dyn NetView (both defined in fedserv-api); the engine's Db
and Network implement them. Reads hand back plain views (AccountView,
ChannelView, ...) carrying only non-secret fields, so the event log, gossip,
and credential material (password hash, SCRAM verifiers) are unreachable from
a module. All command modules ported; behaviour unchanged.

email: optional logo image in the header
A configurable logo URL renders as an image beside the brand name in the
email header (falling back to the brand text when unset). Email clients
need a hosted image, so it takes a URL rather than an inline asset.

button, muted footer) and adds a configurable accent colour alongside the
brand name.

Emails now render an HTML template from templates/email with a plaintext
fallback, sent as multipart/alternative. A configurable brand name shows
in the template. Reset and confirmation mails share one styled base.

nickserv: email confirmation (CONFIRM) on registration
When email is configured and a REGISTER includes an address, the account
starts unverified and a confirmation code is emailed. CONFIRM <code>
verifies it (a federated AccountVerified event); INFO shows an unconfirmed
email until then. Emailed codes now carry a purpose (reset vs confirm).

email: outbound mail subsystem and RESETPASS
Adds a config-driven mailer: a SendEmail action the link layer pipes to
a configured command (sendmail -t, msmtp, ...) off-thread. NickServ
RESETPASS <account> emails a short-lived code to the address on file;
RESETPASS <account> <code> <newpassword> completes the reset. Codes are
node-local; the password change itself federates.

nickserv: grouped nicks (GROUP/GLIST/UNGROUP) and GHOST
A nick can be grouped to an account and then identifies to it; the store
resolves an alias to its account everywhere it authenticates. GHOST (and
RECOVER) rename off a session using a nick you own. Grouped nicks
federate and are dropped with their account.

nickserv: add SET (password/email) and DROP
SET PASSWORD changes your password (derived off-thread like register) and
SET EMAIL sets an address. DROP deletes your account, releases and drops
the channels you founded, and logs you out. A dropped account federates,
and each node logs out any local session that was relying on it.

nickserv: add INFO and ALIST
INFO shows an account's registration date and (to its owner) email; ALIST
lists the channels an account founds or has access on. The UTC time
formatter moves to the db module so both services share it.

Mirrors the chanserv layout: nickserv.rs keeps the service and dispatch,
each command (register, identify, logout, cert) lives in its own file
with its subcommands and parameters. Behaviour is unchanged.

nickserv: log out the loser of a registration conflict
When a peer's registration wins a name a local session was logged into,
that session authenticated against a credential that no longer exists.
The node now clears its accountname (the ircd emits RPL_LOGGEDOUT) and
tells the user why. A new outbound channel lets the engine push these
services-initiated actions to the uplink from the gossip path, drained
by the link loop alongside incoming traffic.

password was wrong, instead of one message for both.

nickserv: accept IDENTIFY [account] <password>
IDENTIFY now takes an optional leading account name, so logging into an
account whose name differs from your current nick works. Without it, the
account still defaults to the current nick as before.

Adds channel moderation commands sourced from the ChanServ pseudoclient.
Tracks connected users' nicks and hosts so targets can be resolved by
nick and banned by *!*@host. All commands require operator access.
